11Oct94: 170th FS 'SI'. Around October 1995 this aircraft was painted with markings for the 186th FS
as the USAF led the unit to believe they were converting to the block 30 at this time. The
air force canceled the plans stating nothing was finalized.
17Nov99: (170th FS) Collided with F-16C (87-0240), near the town of Vermont, IL about 80km north of
their base. One pilot landed the aircraft while the other was forced to eject. Cause is
believed to have been from one of the pilots losing situational awareness. The mishap
occurred as the upgrading pilot performed an air-to-air intercept on the instructor who was
acting as a target and flying slow speed with lights out. A high to low intercept was flown
by the upgrading pilot and he did not achieve sufficient lateral displacement for his Vc
(overtaking velocity). He rolled out behind the target at 500 feet and 180 knots of overtake.
An evasive maneuver was initiated at 200 feet and less than 1 second prior to impact. The
right wing impacted and severed the target aircraft's left horizontal tail and severely
damaged the corresponding hydraulic integrated servoacuator. The aircraft then experienced
total hydraulic failure and went out of control. The instructor pilot ejected safely and the
upgrading pilot flew his damaged aircraft to base.

towed from the Foxtrot taxiway (next to the alternative runway) coming
from the washing area called a "birdbath" when the right wing hit some
of the "birdbath" structure.
The TSgt who was driving for some reason swerved the aircraft towards
the birdbath instead of keeping it in the center of the taxiway. The
structure of the birdbath stuck on the right wing pulling it off the taxiway.
Most of the damage was the result of the nose gear collapsing in contact

Severely damaged 15May11 Panjwaii District, Afghanistan in night landing accident; the
aircraft rolled over during an operational landing.
The night mission called for two CH147D Chinook helicopters to simultaneously insert troops
to an unprepared Helicopter Landing Site (HLS) while two CH146 Griffon helicopters provided
force protection. After arriving on scene, the Chinooks manoeuvred around the high ground
to the south and established themselves on final approach, with the accident Chinook following
the lead aircraft.
As it landed, the lead Chinook generated a large dust ball, which the accident Chinook crew
assessed would not jeopardize their approach and landing. Descending through approximately
30 feet above ground, both pilots lost visual ground references despite their use of night
vision goggles. The landing flare continued until touchdown, at which point the pilots
felt an unusual aircraft motion to the right. Realizing that the aircraft would roll over,
the Aircraft Captain tried to take control and salvage the landing as the rest of the
occupants braced for the crash. Once the helicopter came to a rest on its right side,
both engines were shut down, and the five crew and 26 passengers egressed. One passenger
sustained serious injuries while eight sustained minor injuries.
The helicopter was recovered 17May11 by a USMC CH-53E of HMH-461
Shipped to Canada. Last reported at CFB Petawawa, ONT, Canada. May become a GIA.
